PHILADELPHIA, September 9, 2014 /PRNewswire/ --
"Leveraging Evidence Across the Care Continuum" co-chaired by David Bates, MD, MSc, and Patricia Flatley Brennan, RN, PhD
The third annual Healthcare Leadership Forum, hosted by Clinical Innovation + Technology magazine and sponsored by Elsevier Clinical Solutions has expanded its mission this year to include multidisciplinary evidence-based practice and cooperative care. This meeting brings together physicians, nurses, IT and healthcare executives and leaders to share best practices for driving knowledge to the point of care, on Sept. 29-30 at the Waldorf Astoria Chicago.
"Leveraging Evidence Across the Care Continuum" features keynote speaker Mary D. Naylor, PhD, RN, FAAN, who is Director of the New Courtland Center for Transitions and Health at the University of Pennsylvania School of Nursing, along with a host of other thought leaders. Naylor will dive deep into the evidence of effectiveness of care transitions. This two-day event focuses on leadership, big ideas and fresh thinking as speakers share knowledge and outcomes of interdisciplinary projects.
"If we are going to deliver excellent care, we must do it as teams," said conference co-chair and speaker David Bates, MD, MSc, who is Chief Quality Officer, Senior Vice President, Chief of Division of General Internal Medicine at Brigham and Women's Hospital and Medical Director of Clinical and Quality Analysis at Partners Healthcare. "At this meeting, we will cover care coordination, care transitions and how to communicate with patients inside and outside the hospital. Healthcare leaders need to embrace both evidence and care coordination to provide better and safer care for our patients and reduce costs across healthcare."
Topics to be discussed include: pushing innovation and evidence across the care continuum, using health IT to improve patient-clinician engagement and team communication, making transitions of care safer, establishing mobile health strategies, identifying risk earlier to cut costs and using big data and analytics to more effectively manage patients.
"In its third year, the Healthcare Leadership Forum continues to grow with new, valuable insights from evidence-based practice leaders across healthcare," said conference Co-Chair Patricia Flatley Brennan, RN, PhD, Moehlman Bascom Professor of Nursing and Professor of Industrial & Systems Engineering, University of Wisconsin - Madison.
