The elbow naturally operates as a hinge joint, with the majority of the natural range of motion occurring on one plane, although it also includes a rotational component. The joint involves the articulation of three bones: the humerus, the radius and the ulna. The articulation of the head of the radius on the ulna allows for forearm pronation and supination, resulting in the ability to rotate the wrist.

A total implant replaces both articulating sides of a joint. The two main divisions of elbow implants are semi-constrained and unconstrained implants. Semi-constrained implants have a humeral and ulnar component linked together in a hinged joint. Unconstrained implants are similar to resurfacing implants for other areas of the body and preserve more bone stock.
Although unconstrained implants produce a more natural motion than hinged implants, the potential for instability remains a concern that has limited their acceptance in the market.
Radial head implants replace the proximal head of the radius. The primary indication for implantation is fracture repair, particularly dislocation, although it may also be used for degenerative arthritis. Radial head fractures are one of the most common traumatic fractures seen in adults and typically result from falling onto an outstretched hand.
An alternative to the complete replacement of the radial head is a radial fracture plate, which is not discussed in this report. Activities that place a high degree of torque or twisting force across the elbow joint are not recommended for recipients of total elbow replacement devices. Approximately 90% of all implanted elbow devices are still functional beyond 12 years of use.
